Accurate endocardial activation representation of atria by intracavity inverse mapping
Atrial fibrillation (AF) is the most common heart rhythm disturbance, and radiofrequency catheter ablation has been widely used to treat AF with certain success. However, despite “optimally” combined ablation approaches, the success rate for persistent AF (PeAF) remains low. A major hurdle is that the complex functional and anatomic substrates of PeAF and lacking of effective mapping tools to construct representative electrical activations during AF. To address this problem, our group aims to develop a novel, noncontact, “open”, multi-electrode mapping system to reconstruct panoramic electrical activation in atrial chambers simultaneously by intracavity inverse mapping.
We have developed a novel numerical approach to accurately estimate Neumann boundary conditions on the catheter surface accurately by employing forward solution and numerical derivatives. Then we have integrated the estimated Neumann boundary conditions into a finite element inverse problem package and employed this as a powerful tool to investigate the feasibility and sensitivity of the proposed mapping study.
Our results indicate that the potential-based, intracavity inverse mapping is capable of accurately reconstructing electrical activation patters on the endocardial surface. Furthermore, we have demonstrated that the optimized geometry meshes, diameter and flexibility of basket catheters are important factors for accurate inverse mapping.
